The Central Minnesota Council on Aging has free copies available of "The Five Wishes" booklet and an organizer/workbook for older adults and family members that can be used to organize personal and financial information.
"The Five Wishes" booklet is a living will that allows people to document how they want to be cared for if they become seriously ill and can't make their own decisions. The booklet addresses medical wishes, while taking into consideration the emotional and spiritual needs. The booklet also discusses the importance of pre-planning funeral, cremation and cemetery arrangements.
The booklet was written with the help of The American Bar Association's Commission on the Legal Problems of the Elderly. It meets the requirements under the state law to serve as an advance directive in Minnesota.
